This is a consumer (end user) promotion. Rewards will be fulfilled in the form
of a Prepaid Reward Card in US Funds. The Prepaid Reward Card is not
redeemable for cash and may not be used for cash withdrawal at any
cash-dispensing locations or automated gasoline pumps. Prepaid Reward Card is
non-transferable and non-refundable. Rewards are limited to one per customer,
family, household or organization. QSC employees or members of their household
are not eligible to participate in the rebate. Offer valid from an Authorized
U.S. or Canadian Dealer: Offer may not be available with other rewards,
rebates, offers or promotions. Reward applies to QSC TouchMix-30 model
purchased new (boxed A-Stock) from an Authorized U.S. or Canadian Dealer (not
valid in Quebec) on or between October 1, 2018 and November 19, 2020. Claim
forms must be received no later than December 19, 2020. Request must include
invoice, reward claim form and UPC barcode cut from the package. Submissions
received incomplete or without proper documentation (as determined by QSC in
its sole discretion) will not be honored. Offer valid on purchases made in the
U.S. or Canada (not valid in Quebec) and is open to U.S. and Canadian
residents only (not valid in Quebec). QSC reserves its right to confirm
participant's identification before honoring reward request. Reward cannot
be sent to P.O. Box addresses (except where required by law) or addresses
outside of the 50 United States or Canadian provinces (not valid in Quebec).
Not responsible for lost, late, incorrect, incomplete, damaged, misdirected,
illegible or postage-due requests that are not properly submitted for any
other reason. Maintain copies of all materials submitted for your records.
Employees of QSC or their families, its resellers or distributors are not
eligible nor may QSC resellers or distributors claim rewards on behalf of
their customers. Void where taxed or prohibited by law. Please allow sixty
(60) days for mailing of your reward item. This is a mail-in offer, not
redeemable in-stores. All reward eligibility as determined by QSC is final.
